You may know SameAddress from the helpful stories we share in each issue of ACCESS. You may know us because you’ve talked with a representative of ours at an event or seen us on TV. Or, maybe you know us because you heard about us from a friend. We think it’s important that you know who we are because we’re a program of the AAA 1-B, and in the business of helping seniors “age in place”.
SameAddress is the private-pay arm of the AAA 1-B, and we offer a wide range of services, available to anyone, on demand. As part of the Agency, we are a trusted resource with deep roots in helping seniors maintain their comfort, safety and independence at home, while providing peace of mind for their family and loved ones. What’s more, our clients can feel good knowing that any profits generated by SameAddress go straight back into the AAA 1-B programs to help pay for services for those in need. Here are some of the services we offer:
Homecare and Daily Living Support
We know what it takes to help keep seniors happy and safe at home, whether you need just a little or a lot of help around the house. We can provide a wide variety of services, like:
• Cooking, cleaning, shopping and transportation
• Nursing and personal care
• Care assessments
• Support transitioning home from a hospital, rehab or nursing home • 24/7 care
Home Safety and Modifications
A safe and secure home is key to remaining in your home as you age. Here are some of the ways we help ensure this:
• Home safety assessments
• Senior-safe home modifications
• Home maintenance and repair
• Telehealth and other monitoring technologies
Chronic Conditions
We’ve recently expanded to offer even more helpful services. Using a grant from the Michigan Health Endowment Fund, SameAddress developed
a series of specialized assessments and care plans to help manage long-term conditions, including:
• Memory care
• Fall prevention
• Other conditions, like CHF, COPD, Diabetes and more
Once you become a SameAddress client, you receive access to our dCare portal, which enables you to view the services you’ve ordered and when they are scheduled, billing information, and a wealth of other useful tools to help you maintain your independence at home.
